No CallRoleThe Registered Nurse (RN) Case Manager provides,
coordinates, teaches and supervises the nursingcare of Hospice
patients and families/caregivers. Working in collaboration with
other members of theinterdisciplinary team/group, the RN Case
Manager plans and manages the nursing component of the hospice
caseload. The RN Case Manager makes initial and ongoing nursing
assessments, judgments and treatment decisions based on patient and
family/caregiver needs and wishes and in accordance with her/his
professional skills.Supervisory ResponsibilitiesThe RN Case Manager
is responsible for supervising LPNs, CNAs, and Patient Care
Volunteers.Experience RequirementsThe RN Case Manager must have at
least one year of staff nurse experience in a comparableEducation
Requirementssetting/unit. Hospice and/or palliative care experience
is strongly preferred.The RN Case Manager must have a degree in
Nursing from an accredited nursing program. A BSNdegree is
preferred.Required Certifications And/or LicensesThe RN must have
licensure from the governing body for practice for the District
ofColumbia, State of Maryland, and/or the Commonwealth of Virginia,
based on thestate/jurisdiction of practice; or licensure from
compact state. Healthcare Provider BLS (CPR)is required to practice
in the State of Maryland and the District of Columbia. A
CHPNcertification is a plus.Overall SkillsThe ability to
demonstrate superlative oral and written communication skills as
well as ability toorganize and manage projects within defined
timeframe and budget; work both independently and aspart of a team;
demonstrate excellent interpersonal and collaborative; demonstrate
organizational skills with ability to delegate and follow up;

perform a thorough (telephonic) nursing assessment; recognize
stress caused by working with dying patients and their families and
seek to reduce stress through available support and personal
resources; professionally manage highly confidential

peer motivation,problem solving, and who can demonstrate
initiative, follow-up and follow-through; work in a fastpaced

oral form and deal with problems involving several concrete
