Job OverviewInova Fairfax Medical Campus is looking for a dedicated Registered Nurse (RN) Patient Care Navigator to join the Heart Failure team. As an RN Patient Navigator, you will play a pivotal role in supporting heart failure patients by acting as a liaison, patient advocate/educator, and primary point of contact. In this position, you will conduct thorough assessments and collaborate with patients to develop personalized case management plans. Anticipates patient's needs and advocates for patient rights and appropriate utilization of services.* Conducts comprehensive assessment of the patient's health and psychosocial needs and develops a case management plan in collaboration with the patient and their support system.* Educates patient, family or caregiver(s), and members of the health care delivery team regarding treatment options, community resources, insurance benefits, self-care management, etc. to facilitate timely and informed decision-making throughout all phases of the continuum process.* Manages complex cases effectively and efficiently. Creates a seamless flow through the health system to facilitate the patient's care. Collaborates with multidisciplinary teams to address patient questions and responses about the care process and treatment options.Additional Requirements:* *Work schedule*: Monday thru Friday nights, 8:00 a.m. 4:30 p.m. ***Travel required between Inova care sites.* *Education: *Bachelors degree in nursing (BSN).* *Experience*: 3 years of Registered Nurse experience.
