Post Acute Care Opening in Lancaster, PA Philadelphia 79mSeeking a primary care physician to do rounding in skilled nursing facilities in Lancaster, PA. Full-time Monday - Friday Flexible weekday schedule Perform primary care in a post-acute setting for short term and long-term patients Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care Order and interpret lab results Prescribe and monitor medications Consult with physicians and coordinate patient care with other healthcare practitioners Educate patients and their caregivers on coping skills to handle chronic and age-related conditions Promote healthy lifestyle changes Compensation & Benefits Estimated compensation range of $210,529 to $271,529 annually with no cap on productivity income potential.
