This position provides an opportunity to work closely with patients in their own homes, focusing on rehabilitation and sustainable physical therapy tailored to individual needs. As a vital part of our community-focused health services, you will play a key role in helping patients achieve their highest functionality and independence after injury or in managing chronic conditions. Assess patient needs and craft individualized rehabilitation programs. Implement treatment plans using a variety of therapy techniques. Monitor patient progress and make adjustments to interventions. Educate patients and families on treatments and in-home therapy strategies. Maintain detailed notes and documentation for each patient. Collaborate with healthcare professionals to discuss treatments and patient progress. Proficient in latest physical therapy techniques and protocols. Strong interpersonal skills for effective communication with patients and families. Ability to work independently in a home-based setting. Excellent organizational and time-management skills. Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Valid Physical Therapist license in the state of Maryland. CPR certified.
