MUST:
Experienced Housing Specialist
1+ years of experience as a Housing Specialist
1+ years of experience working within the HIV/AIDS community
Working knowledge or understanding of tenants' rights and
responsibilities
Working knowledge of community resources
Working knowledge RW policies and procedures
Excellent communication skills
Must possess strong organizational skills and be detail
oriented
Must be able to lift, carry and handle equipment, supplies and
other work site materials based on position duty requirements
Must demonstrate good internal and external customer service
skills
Ability to maintain complex record keeping, budgeting, and
mathematical skills
Ability to produce required reports to federal and state
governments and funding sources
Ability to work collaboratively with other Care Management
employees and service providers of many professions
Fluency in Spanish highly preferred
Obtain and keep HUD certification as a Housing Counselor within 3
years of employment
Bachelors Degree in Human Services or a 2-year degree in a Human
Services related field preferred
DUTIES:
Assess housing barriers of individuals and families experiencing
homelessness to determine housing and service needs.
Determine that financial requests are legitimate and adhere to the
rules and regulations set forth by the Ryan White (RW) grant
program, administered by the Health Resources and Services
Administration (HRSA), and any additional requirements
Develop a housing procurement, financial, and self-sufficiency case
management plan with clients. This shall include completion of all
RW required assessments and forms.
Provide mediation and advocacy with landlords on the client's
behalf to develop a workplace plan to obtain and maintain
housing.
Assist participants in locating and securing housing per RW grant
standards.
Maintain consistent communication channels, both written and
verbal, between several parties, i.e. tenant, landlord, referral
source, collaborating agencies, debtors, and creditors.
Provide information and referral assistance regarding available
support from appropriate social service agencies and/or community
programs. (e.g. Northern Virginia Family Services for other
potential housing programs not covered by RW)
Assist in development of and encourage adherence to a personal
budget through pro-active housing and budget counseling sessions;
provide budget counseling and education to assist clients in
establishing payments plans for bills and past debts to assist
clients in obtaining and maintaining their housing.
Assist participants in development of a
strength-based/solution-focused individualized goal and action plan
that promotes permanent housing and self-sufficiency; develop an
effective, timely referral network to ensure ongoing direction and
support as needed.
Identify participant strengths and barriers to stability and assist
participants to reducing barriers and linking to resources and
services.
Provide proactive follow-up home visits to ensure stability and
further progress towards self-sufficiency; this includes support,
advocacy, reducing isolation, listening, problem solving, and
identification of resources to assist with reintegration of
participants in the community.
Observe the dignity, confidentiality and privacy of patients and is
aware of HIPAA guidelines.
Apply knowledge of residential lease contracts to educate clients
regarding their rights and responsibilities.
Collect and report program data, including, but not limited to, the
Housing Management Information System (HMIS) reporting and funders
required data.
Maintain a complete working file providing activity documentation
and copies of all corresponding paperwork.
Request, process, and mail monthly checks to landlords.
Successful applicant will obtain Virginia Housing Counselor
certification following two years of employment with this
agency.
The successful applicant will obtain trauma informed care,
motivational interviewing, and harm reduction training in the first
year of employment.
Obtain and keep HUD certification as a Housing Counselor within 3
years of employment.
Conduct community outreach, build relationships with lenders,
realtors, and other professionals to develop referral sources and
promote program awareness.
Assist with facilitating education workshops, including scheduling,
marketing, and procuring speakers.
Perform other duties as assigned
